Illinois Joining Forces to Accept Requests to Administer Courtesy Wellness Checks for Veteran or Service Member
ILLINOIS —As part of Illinois Joining Forces’ (IJF) ongoing celebration of the upcoming Memorial Day Weekend, IJF has announced that it’s Care Coordination Center services will expand to administer courtesy wellness checks upon request for any and all IllinoisService Members, Veterans and their Families (SMVF) during this time of rapid change, transition, and uncertainty.
IJF is a statewide, public-private network of military and veteran serving organizations working together to improve services for Service Members, Veterans and their Families (SMVF). The IJF Care Coordination Center is open from Monday through Friday from 8:00a.m. to 5:00p.m. CST.
To request a wellness check for you or your Veteran, service member, or their family member please dial: 833-INFO IJF (833-463-6453) and a Care Coordination Center professional will customize and coordinate your high-touch, holistic experience — unique for your needs. This can be a single or ongoing wellness check, navigation support of Veteran Services Organizations, and more.
